Important Records And Papers To File

Keep Records Together! Document Everything! Include Phone Calls!
(Be sure to write down the date, time, person spoken to, and information received!)

Some of the records that you may need are:

Personal, including:

  • Birth certificate
  • Social Security card
  • Guardianship orders
  • Family information (who to contact in case of emergency)
  • Records from agencies who provide services for you
  • Selective Services Registration card (for boys 18-years old)
  • Income information (including loan notes, stocks, bonds or other investments)

Medical, including:

  • Medical insurance card
  • Names, addresses and phone numbers of doctors, dentists and therapists
  • Immunization record
  • List of medications you are taking and prescribed dosage levels
  • Dates and results of any surgeries or medical procedures; or, emergency room visits
  • List of allergies
  • Specialist and therapist reports

Education, including:

  • Copies of Individual Education Plans (IEPs)
  • Educational testing reports
  • School progress reports and report cards
  • Awards, certificates, diploma, transcripts and contact logs

Vocational, including:

  • Reports from vocational assessments
  • Vocational courses taken
  • Work record including dates, contact persons and phone numbers
  • Letters of reference
  • Your resume
  • Job Portfolio
